Exercise For Chemo Brain: Can It Improve Memory and Focus?

Exercise for chemo brain is backed by early evidence: a randomized trial from UC San Diego linked a 12-month exercise program to better attention and memory in breast cancer survivors. The results are promising, but larger studies are still needed.

What is chemo brain?

“Chemo brain” describes the thinking and memory problems that often follow cancer treatment, such as memory lapses, trouble concentrating, or difficulty finding the right word. Doctors call it cancer-related cognitive impairment. One in three breast cancer survivors experiences cognitive problems, and for some people they last long after treatment ends.

Can exercise help chemo brain?

Two lines of research suggest it can.

After treatment. The UC San Diego trial, published in the Journal of the National Cancer Institute, included 253 survivors who had finished treatment, were inactive, and reported cognitive difficulties. The exercise group followed a tailored 12-month program delivered by phone and email, and showed greater improvements in attention at six and 12 months.

Before and during chemotherapy. A separate analysis of 580 U.S. breast cancer patients and 363 people without cancer found that patients who met recommended exercise levels before and after chemo outperformed those who never did. Those active before chemo scored similarly to the control group on memory and attention tests.

How can exercise help the brain?

Researchers are still investigating. Possible explanations include:

  • Slower biological aging. A companion study suggests exercise may slow epigenetic aging, though a direct link to cognition is unconfirmed.
  • Brain health. Aerobic exercise is known to benefit cognition and brain health in older adults.
  • Better sleep, energy and mood. Fatigue and poor sleep hurt concentration, so improving them may help indirectly.

What exercise should you try?

Most research focuses on moderate aerobic activity: brisk walking, cycling, swimming, or low-impact dance. You don’t need to be athletic. Participants in the UC San Diego trial were previously inactive.

How to start safely

  • Talk to your oncology team first.
  • Start small, even 10 minutes of walking.
  • Build up gradually over weeks.
  • Stay consistent, since benefits appeared over 6 to 12 months.
  • Track energy, focus and sleep in a simple journal.

Other ways to support memory and focus

Exercise works best as part of a wider plan. Many survivors find it helpful to keep a planner or phone reminders, do one task at a time, protect sleep, and take short breaks during mental work. If symptoms are affecting work or daily life, ask your care team about a referral for cognitive rehabilitation.

What we still don’t know

Researchers say larger studies are needed to confirm these findings and to learn how exercise might sustain improvements over time. Exercise is not a guaranteed fix, and it works best alongside good sleep, routines and support from your care team.
